Incident reports are faxed immediately and are also available 24/7 on our web site. Incident reports are only $5 and include:

• Number of the truck being reported
• Date, time, and location of the incident
• Road conditions
• Weather conditions
• Traffic conditionsv
• Incident description
• Name and phone number of the person reporting the incident
